| Fee for Services |
| Our mission is provide the best gastric band care to our patients with the convenience of weekend hours at an affordable and reasonable price. In the event that you find yourself to be too tight and it is in the middle of the week, you must go to your surgeon’s office, where you will be responsible for a regular office visit fee. Access Weight Loss Care LLC, is a separate entity and not affiliated with any hospital. |
| As a condition of receiving care at access wt-loss care, you must agree and abide by my expertise and recommended treatment plan. Trust that I have your best interest in mind and want you to succeed. There are certain situations where a patient might come in for a visit expecting a tightening but the appropriate treatment would be a loosening of the band. The same occasionally occurs that a patient comes in for a loosening and paradoxically, a tightening my be the appropriate treatment. Sometimes, the band is perfectly adjusted and does not require an adjustment. The adjustment is determined by your clinical presentation coupled with my experience in caring for gastric banding patients. Please do not try to negotiate your band adjustments or attempt to receive “free” care because the treatment was not what you expected. |
| Insurance Policies |
| We will work with all insurances. Please call the office and leave a message for one of the insurance specialist, Sandra or Maria, in order to find out if your insurance provides you with bariatric coverage. If it is determined that your insurance does not have bariatric coverage, you will be responsible for the adjustment fee. |
| Initial Consultation There is a new patient process in order to become a patient at Access Weight Loss Care. You must complete a patient demographic form and submit a front and back copy of your insurance card. You will also require a new esophagram to establish a baseline reference point of the position of your Lap Band. You will also require a copy of your operative report. If you had a recent esophagram, within the last 6 months, it might be acceptable. The final determination will be based on your clinical symptoms.
In order to help answer some preliminary questions, Yes! I am still the Nurse Practitioner with Dr ChrisitineRen-Fielding, Dr. George Fielding, and Dr. Marina Kurian at NYU’s Weight Management, I have not left them nor do I intend to leave them. I am vested in all my patients. In order to expand my capability and access to care and because of patient demand for me to accept insurance, I needed to collaborate with a physician in New Jersey. I could not have been more fortunate and honored to call Dr. Stefanie Vaimakis my collaborating physician. Dr. Vaimakis is a highly experienced laparoscopic and bariatric surgeon and most importantly…she truly cares about the patients, making me the luckiest Nurse Practitioner in the world. I am part of the best bariatric teams in New York and New Jersey. That also means that I will be able to expand access to care to more patients. |
